Mod Post The future of this sub NSFW

After this sub was "accidentally" banned 2 days ago there's been a lot of discussion about the future of the sub.

Whether it was an accident to not, the possibility exists that this sub and others will be banned from reddit in the near future. In the event that happens what do we do?

I started as a mod here when the sub had only 3k members and my intention was to grow it to where it is today, and more. I last wrote about how the sub is moderated in 2022.

In principle, it would be better to have an trans resource site independent of reddit and corporate control. In practice its very difficult to achieve for a number of reasons

There's no point in moving to another site like Discord which is susceptible to the same risks as reddit. i.e. based in the USA. But what other sites are there, and where else is safe in the long run? Not just safe from hostile governments, but whoever runs the community losing interest, or data (susans.org lost years of it with a hard drive crash), selling out, etc.

Neither Discord and Facebook are indexed by search engines making it difficult for people to discover the resources in the first place, or finding information once you're there. It's like a black hole for knowledge; you put it in and it disappears. Personally, I'd never waste my time on building this kind of community on sites like that.

Reddit also provides, or did, legal protection. If a surgeon doesn't like what's posted here they can't easily censor it. And especially important, they can't attack me personally as its not my responsibility. Good luck going after reddit corporate.

As one of the largest social media sites in the world reddit makes it easy to build community, there's so many of us already here. People have mentioned sites like Lemmy as alternatives, but as far as I can tell they have tiny membership and few people have even heard of them.

A major advantage for me was reddit's wiki's. Few subs take advantage of them, but I believe its a great way to build and spread knowledge, and it has helped build this sub and raise the general level of knowledge. People have asked that it be copied off site, but if this sub disappears many of the links in the wiki will also disappear. Its not nearly so useful at that point. I don't think anyone else will build or maintain a wiki either, as it seems to interest very few people.

Regardless if reddit banning this sub or not, I'd like to see another site even better than this one, but I'm not sure its possible. Even more so while reddit hosts trans content as 99% of people will just come here anyway. Reddit basically killed old style forums years ago and nothing's changed since then.

It's even more difficult to build a trans surgery surgery community on another site while this sub exists because its so big and useful that almost no one would bother going there. And I'm not shutting the sub down to force everyone to move to another site. That would cause immediate harm to people who use the sub.

If this sub does get shut down I personally won't be trying to rebuild elsewhere. I'm burned out with this and don't have the energy.

If anyone wants to discuss how to build a successful trans surgery community I'm willing to offer my advice. I'd like to see it happen and it would be great if people had a place to go, and knew about it ahead of time. My main aim is to help people, and it doesn't matter to me where that comes from.

Edit

If you set up any external resources for surgery, hrt, etc please add them in the comments here. And I suggest people save the links in case this sub, or worse, all trans content on reddit disappears.

There’s a number of people talking about off site projects they are considering or actually doing. Persons you could get together and discuss if you could work together.

This looks interesting r/RedditAlternatives

There's some cisgender people wanting to comment here in support of Lemmy and other reddit alternatives. Rule 5 limits cis people on this sub, but I'll allow it on this post only and give them a flair "cisgender reddit alternatives". If you're one of them please don't comment elsewhere.

Lemmy Discussion

Lemmy keeps getting mentioned. I don't know much about it yet. Its pitched as Fediverse reddit replacement.

According to the statistics here Lemmy has 477,049 total users and 45,194 monthly active users. The trans instance https://lemmy.blahaj.zone has 8671 total users and 971 monthly active users.

This sub alone has 93,419 members, and in the last 30 days 4.6M views, an average of 20.2k daily unique visits, 4.0 subscribed, and 1.2k unsubscribed. The main FTM surgery subs in total have about that again, and the HRT subs are a bit larger in total.

This sub is then 10 times the size of the main trans Lemmy instance, and the total with the subs I mentioned is approaching the entire size of Lemmy. This doesn't include all the very main trans subs which are individually many times larger as I only included the important medical subs.

I have a few reservations about Lemmy, partly because I know so little at this point

  • Can Lemmy can scale to the size required if trans content was banned on reddit.

  • I couldn't find much information on Lemmy's moderation tools. Currently this sub attracts a lot of hate and chasers, which moderation easily takes care of. In the past the have been excessive amounts, but reddit has cracked down on it, and provides tools to limit it (not very good ones). Lemmy would be unusable without this.

  • Lemmy works by sharing data across multiple instances (computers) and it appears there seem to be privacy concerns about the amount of data on users that is shared.

  • What is to stop the owners of the instance shutting it down, or the data being lost for any other reason? Although not a corporate it makes no difference. There would be a massive loss of knowledge and history.

If anyone has expert knowledge on Lemmy I'd be interested in learning more.

The author of the Engadget article on the sub's ban made a YouTube video on the Fediverse

BA & FFS with Dr. Salam Kassis ~1 Year Later (23y/o) NSFW

Hi! I wanted to share my FFS experience with Dr. Kassis in case it helps anyone. (I also had BA and he lives up to the hype which is how I found out about him, but I won't be posting pics of that here!)

He used to be with Vanderbilt in TN when I did mine back in late 2025. The reason I chose them is because they were the quickest out of 10 places to get an appointment with before my BCBS insurance switched over to Aetna. (~1mo consult, ~3mo surgery after that)

He's now doing work at Kassis Salameh Plastic Surgery in Kentucky. His specialty is rhinoplasty.

My timeline is as follows:

Oct 2025 - Breast Augmentation, OTM, 290cc

Nov 2025 - FFS Stage 2, Chin (Was going to be trach shave/jaw too but I opted out.)

Dec 2025 - FFS Stage 1, Invasive nasal surgery (German genetics), brow bone shave, hairline advancement, brow raise.

I write the stages like that because my Stage 1 was scheduled first for December and they slid the Stage 2 in for November. This may be totally false but I think getting chin/jaw done last might sag the skin more (it looked like that initially) so the surgery order almost acted as some sort of a minor facelift which was cool!

I love love love my results. I think they speak for themselves and he had a difficult base to work with so that's a testament to his quality. The nurses were also very kind! I am happier and would do it all over again. Sad what happened to Vandy but such is the environment these days. Most trans friendly hospital I've been to personally.

Not much pain as well! It took about 30-50 days for most feeling to reappear in chin/head.

I also employed the help of a trans surgery caretaker for the latter two surgeries, T4T caregiving- they were expensive but very helpful in assisting my during surgery!

Nitpicks:

Symmetry with hairline/chin bone - Looking at photos before, I definitely had some asymmetry, and I would have liked that to be fixed but I'm not sure if he thought it would throw off facial balance. It's still better than before. Though, and I noticed this initially- my chin bone definitely feels a bit like there's a dip on the right side compared to the left, but with more bone on the right side. For the forehead, it's the same deal with it being higher/having a more noticeable break in hair than the left side. It could be the angle the surgery was done at.

Taken as a full package, I'm really happy with my FFS and it was a definite step up from previously. Hopefully an additional account of his work is helpful to anyone considering him!

This post is meant solely to provide reassurance to those who are pre-op for FFS and have already scheduled their surgery or are considering doing so. This is not meant to advocate for or against choosing any particular surgeon.
Please ask your surgeon all the questions you need to feel comfortable before proceeding with surgery. This is simply my personal experience and my results.

Before having FFS, I tried to research my surgeon, Dr. Nima Khavanin at Johns Hopkins, but I couldn’t find anyone discussing his FFS work. This made me extremely nervous, but I ultimately decided to go through with it.

I’m still healing, of course, and I have a long way to go before my results can truly be judged. However, I cannot stress enough how happy I am with my results so far and with my overall experience. I have had almost immediate dysphoria relief. Of course I still experience it but there are days now where I genuinely experience gender euphoria now. For the first year of my transition I never once was gendered correctly by a stranger and in the past 9 weeks I can count on one hand the amount of times someone has misgendered me. It’s been such a breath of fresh air.

I still encourage everyone to do their research, ask all their questions, and make sure they choose the surgeon who is best for them. Of course, everyone’s experience and results will be different.

So, if you’re in the same position I was a few months ago, here are my pre-op and post-op photos. I hope this helps someone!

Procedures done

- Type III forehead setback / frontal bossing reduction
- Brow lift

6 month update to my SRS recovery. I had the new method in April 2026.

I'm not thrilled with my aesthetics. Sensation is just okay, and my clit is very close to my urethra, making access during partnered sex difficult.

I am having a revision with Dr. Pang in Decmber 2026.

Im 2 years post op. I went to Dr Brandt in reading PA and im thinking i might get a revision or just a labiaplasty but im not sure , my clit isnt really that sensitive either. During sex i experience pleasure but never gotten to an orgasm and same with masturbation. I dont really know what to do about not being able to orgasm so far, i thought maybe with time it would get better but it seems ive reached a stand still. Im happy about the surgery but i feel aesthetically it could look better in some pictures i feel good other angles i do not and i feel like my labia is loose. and function wise i just really dont experience that much pleasure. Ive seen some pretty good results on here and its hard not to compare. Any advice/thoughts?

i feel a need and desire to continue this journal. today is actually the most mentally sound I have been my entire recovery. I was operating on a delusion that I could only be positive, and that has made this small negative 10 times more devastating - ignoring the insane shifts in my social circle I've had to do 😖 but as a practicing somatic health care worker..... how could I lose sight of the balance needed in somatics. there have been excruciating mistakes I've made in these past (almost) 3 weeks. but like I've been saying... it's day to day, minute to minute. so while i recalibrate, my body must rest, and my mind has to slow down as well. Bipolar is an intensely difficult mental landscape to be introducing something as... intense... as vaginoplasty to, so hunny.... I did not prepare myself for the storm. the clouds have passed and the sun is shining again, now I just have to mend where I can, and keep on keepin on. sending love and well wishes always.

Hello all, I would like to kindly know your take with the bed pads that I am constantly using with every dilation I do which is 2times a day which is starting to get pretty pricy as I am now at mq 11th month. Is there any other approach for this? I have no clue how else should I do it as I always dilate in bed and it always gets messy. How do you guys do it? 😊 And sorry for my English..

Jolts and zaps in my new vagina NSFW

I got SRS 6 days ago and I am constantly feeling jolts, zaps, and something akin to that vertigo feeling you get on your stomach? Except that I’m getting it in my vagina. They don’t particularly hurt, but they catch me off guard and make me flinch a little

Is this supposed to be a good thing?? Nerves reconnecting and stuff?

Low profile / mini breast implants? NSFW

Considering moving forward with very small implants (Motiva 205cc ergonomix, mini),
Has anyone gotten mini implants like these?

One of the big reasons I'm considering this small is because I'm already insecure about the width of my chest from the side, and I'm afraid that any larger will make me look top heavy in a way I don't want. I also like the "sloped" small ballerina boob look with no upper fullness.

I of course don't want to feel like I'm going through surgery for not-enough-change either.

For girls who have gone through with their breast augs:

  1. How sure were you of the size/look you wanted?
  2. Do you have any regrets? Bigger / smaller, Different profile?

Any help would be greatly appreciated. Thank you

One week post op.. NSFW Spoiler

I have had an very interesting week. Gonna be pretty graphic in describing the week.
Pain was pretty manageable at the hospital but when I discharged I didn't have the best bed at my rental so I had to frequently get up to relieve pressure from the packing and urine. Most pain was really from the packing and catheter.
I'm going to really state how important ice packs were because I woke up often in bladder pressure sweats.
If you can be kinda tilted or remember to get up often from bed to for the first week, no matter how uncomfortable.

When I had the packing taken out it was stuck a weird feeling. Like a tickling down the throat but in a vaginally canal.
Now having the catheter taken out was absolutely bliss. Peeing afterwards shucked tho was dripping all over and spraying lol.
I'm Soo much more mobile after catheter and packing removal it's insane.

Is this type of result achieveable for me with fat grafting to the hips and thighs? NSFW

Pretty much the title haha, first 4 pics are a rough idea of what I wanna achieve and the last 1 is my body when I was 65kg (I've gained a little weight to prepare for fat harvesting but overall about the same, legs jst look a little skinner cuz I don't gain there unforch :/)

is this kind of result/shape achieveable with lipo + fat grafting to the hips and thighs? I'm really worried about it looking unnatural and overly filled (kinda like 2016 kardashian era BBLs) from trying to force lateral width on the hips

Pre-Op Consultation with UofM in 6 Months. Any tips? NSFW

• Upvotes

Hi friends,

For context, I've been on HRT for about 3 years (26 MtF) and I'm getting Vaginoplasty with The University of Michigan. I do not know for sure who my surgeon is going to be quite yet, but I know it will likely be either Dr. Kuzon or Dr. Marquette (hoping for the prior and you'll see why below). For this I had a few questions:

  1. What are the post-operative rooms like? Are they at least decent? Do you get your own room to recover in? I don't know if I could possibly be able to exist in this scenario with another stranger in my room but I also know that hospitals are under a lot of stress as of late.
  2. What is the recovery timeline usually look like? Should I take 2 or 3 months off work to be safe? I wanted to know so that I can appropriately budget for this time.
  3. I've not seen too many before and after photos outside of the top posts regarding the University of Michigan. Because of this I know that Dr. Kuzon is an artist, but the only thing I've seen regarding Dr. Marquette is a botched SRS that happened a few years ago.
  4. What items should I bring to the hospital with me? Right now I just have a few things, my stuffed sharks (gotta bring the chuds), my laptop, phone, chargers, books, etc. But is there anything else that I should be remembering to bring with me for the stay? Something you wish you'd brought with you to the hospital that wasn't on the list of things?
  5. I know the food post-op is pretty bland all-in-all, but is it at least decent? Food is a big part of my life, and I know that toast afterwards is going to be great regardless, but please tell me it's at least decent....
  6. Is there any part to this that I've missed, tips you wish you'd known going in, or things you'd wished you'd known about before going under the scalpel? If there's anything like that please let me know!

If I can think of any other questions, I will add them to this post as an edit. I just saw that it had been well over half a year since the last major University of Michigan post and wanted to know if there is any updated information I can get!

Orchiectomy with Dr. Kim Kuylhee NSFW

On July the 8th this year I decided to get an orchiectomy. Although I’m American, I work in Japan and I’ve been here for several years, but due to a life circumstances, I was going to have to return to the states.

This really terrified me, and I wanted to make sure, whatever the future, I couldn’t be detransitioned by whatever dysfunctional theocratic government is running the country. So I started looking into bottom surgery and orchiectomy locally in Japan. There’s only about one doctor that I could get in at here, which was Dr. Saegusa at the TG clinic in Ebisu. At the time I thought I could be back in the states within a month and I needed the surgery before that. Dr. Seigusa didn’t have any openings for anything less than 45 days out and was charging about 3,000 dollars.

Also, many of the doctors here in Japan are very old-school, their techniques as old too (which isn’t bad, they certainly worked in the past) but trees kind of surgeries have changed rapidly in even the last few decades. I started talking to Dr. Shin Ikebukuro for recommendations. Dr. Shin runs personal health clinic in Yokohama and Ueno, and also moonlights in several other places treating transgender patients. He recommended his friend, Dr. Kim, who works out of Sacred Heart memorial hospital.

I reached out to their organization, and her husband, Justin, reached out for interview. I told him my timeline, he spoke with Doctor Kim, and she scheduled me for the morning where she had an opening about two weeks later. The charge was only 2500USD for an orchie. Justin took care of everything for me, even drove me to my appointments. The first day in Korea, he had forgot to mention that the outlets were different here than in Japan, and so he brought me to his house, I met Dr. Kim who was really so nice. She even introduced me to her cats and gave me a Korean charger/power bank so I could charge my phone. It was such a friendly vibe.

I met with Dr. Kim several times when I was there. Since she knew I wanted bottom surgery as well, she gave me a consult while I was there. She actually is the head of her own gender care unit at the sacred heart hospital, and had nearly all the restrooms there converted into gender neutral. It was a really welcoming place. She discussed her techniques and showed me examples of her bottom surgery work, and I have to say her attention to detail was amazing. I really like her work on the anterior forchette forchette, which I feel western surgeons don’t seem to care about, so the bottom of the vagina just sort of gaps open. She also said she hides the scars between the folds of the labia, which was interesting. Also, unlike most American doctors, she requires you stay in the hospital during the first week or more until the swelling is controlled, and you’ll be connected to some kind of vacuum machine to keep the swelling down (I don’t remember what it’s called)

She mentioned a lot of revisions occur because of wound separation from swelling, so that’s critical to her end result.

She also discussed assessing my available skin and recommended penile inversion initially, as she said of there are complications, we can take additional grafts using peritoneal tissue, and if there are further complications, as a last resort use jejunum. She explained that if you start with jejunum, and dilation doesn’t work, or there’s other issues, there’s simply not many other good places to graft from.

One of the things that wasn’t great was that the rest of the hospital outside of the gender unit is very gendered. Unfortunately at this time my passport was not updated, so although I was in a gender neutral ward, the name on the door was my dead name’s initials with my birth sex next to it. Dr. Kim says she’s been working to get this fixed, but since it’s a shared ward, it’s been a battle with the rest of the hospital for her. But she made sure everyone I interacted with used my actual name. And the nurses were great on the ward.

The next day or so I checked into the hospital room. I couldn’t eat/drink for 12 hours before bed, and in the morning they wheeled me to the OR. I was out with an hour or two. It was extremely fast, felt like I just fell asleep for a second. I was given a radical orchiectomy, where the cords were cut somewhere in my abdomen or inguinal canal. The suture job was amazing. It’s been several months but even right after surgery I couldn’t really see where she had cut, besti can tell she cut right on the perineal raphe, and I had to feel to find where the sutures were. It was a really clean job.

Justin picked me up about six hours later, by then I was walking. He took care of checking me out of the hospital and told me to have a seat. insisted on walking because I thought I was fine. Nearly passed out in the lobby. Just before I was about to black out I said something like “I think I’m going to pass out…” and Justin literally caught me and helped me walk to a seat. By this time we were pretty good friends. I guess I was a bit nonchalant about the way I said I was going to pass out, and said it with about the urgency in which someone might say they are grabbing a coffee.

After fanning me back to life and giving me water, we walked to the car feeling much better. Dr. Kim met me in the parking lot and hugged me and asked me how I felt, told her husband to take care of me in a kind of motherly way, she’s incredibly sweet, and Justin drove me back to the hotel. There was some discussion later on about how she had evaluated my available skin for bottom surgery and said I’d do fine, and I talked with Justin about coming back in a year, prices for bottom surgery was about 21K USD, and since my US insurance no longer covered anything thanks to Trump, that’s about as good as it gets.

Anyway, I had a great experience. my only complaint is the food in Korean hospitals was too spicy for me. I’ll probably ask for a meal modification next time. As for the surgery, there’s was some initial swelling and it was sore for about a month, but nothing that prevented me from getting around (except the first week or so, felt like i needed to take a lot of breaks, almost like fatigue as my body healed). I had to take it easy for about three months for lifting things. At at this point, almost 4 months later, I’m complete healed. I have no scars that I can see, and I really can’t wait to do bottom surgery with her next year.

Anyway that’s about it. I’m open to any questions. Justin, if you’re reading this, you have to go to the drag show down the street from my hotel in Itaewon with me when I come to get my kitty next year 😭

And for those wondering, there’s a nice gayborhood, red light distract and club district in Itaewon where the hotel was recommended by Justin. Fun place, so after hours we’re also very lit, wish I had spent more time there, but I had to get back to work.

Options for post surgery transportation? NSFW

Hi I have a surgery coming in a couple days, the only hitch has been securing transportation. The surgeon is a couple hours away from me, I can get there just fine it’s getting home after the surgery that’s the problem.
Money is tight and I don’t really have family that I can count on.
I did have a friend who was going to take me back but she might have to bail.

I am trying to work with my insurance to see if they would provide a caregiver for the drive back especially since this doctor is the closest in network person in my state, but they haven’t been entirely helpful.

I’m just trying to see my options.

I could maybe just get transportation to and from a train or bus station and ride it home but that is super iffy.

Either way I have no idea and I’m starting to worry I will have to cancel.

(FFS) Who would you go to if insurance wasn't a consideration? NSFW

I'm in a red-state with an insurance policy that explicitly does not cover FFS (no possibility of LOA either). I am planning on paying out of pocket. I started saving up the month before I even started HRT 6 years ago knowing that I would want FFS. I will be putting a deposit down soon for a well-known surgeon. Most posts I see these days are people picking surgeons based on their insurance. Who would you go to if you were in my situation? I can only do this once really since this will blow away 6 years of work, so I want to get this right the first time.
