I'm 32, have had two breast augmentations in the last 10 months, and am now dealing with capsular contracture and new pain. I'm torn on how to move forward. Timeline and details below (long post)
Dec. 2025 – Primary augmentation & Animation deformity
I had a breast augmentation with crescent lift (335cc Allergan high-profile implants, UTM) performed by a well-known, board-certified plastic surgeon. I was very happy with my results.
At my 3-month follow-up in March (with his nurse, not the surgeon), I was cleared for all normal activity. Within weeks, I noticed the outline of one implant along the bottom of my breast. In the following few weeks, I developed a significant animation deformity in BOTH breasts. No pain, but very noticeable distortion when moving/flexing.
In May at an in-person follow up, my surgeon agreed this wasn't an acceptable outcome and recommended converting to OTM placement with GalaFLEX mesh. My goal was to look exactly the same, just without the deformity. I was told there was no urgency and I could schedule the revision at my convenience, so I considered waiting to avoid recovering in the summer. However, the surgical coordinator seemed visibly shocked by my photos and encouraged me to proceed ASAP. His office covered the ENTIRE revision, including new implants and mesh. They fit me in within weeks.
Despite being disappointed about having to do this all over again, I appreciated how seriously the office took my concerns, their willingness to cover the costs, how quickly they addressed everything, and the genuine surprise in the staff’s reactions. (I previously worked in an admin role for a different plastic surgeon, so I was especially observant of the staff. I took it as an indicator of how infrequently they come across this at their practice, but who knows) However, their response made me wonder if he felt the original surgical approach had been a mistake given my anatomy and goals, or if there was a complication in the OR, or if I was cleared for activity too early, which wasn’t caught because the doctor was not present at my final follow up. Regardless at the time I left the appointment feeling reassured and eager to put it behind me.
May 2026 – Revision augmentation
I received 365cc Allergan implants (I believe moderate-plus profile), OTM with GalaFLEX. Recovery was significantly more painful and prolonged, including shooting nerve pain and a squishy/noisy area that concerned me. I requested an extra follow-up at 4–5 weeks, but my surgeon examined me and said everything was normal. The office was accommodating with pain medication refills. Aesthetically I was even happier with these results for the first two months!
Aug. 2026 – Capsular contracture
Around 10 weeks post-op, I noticed the rounded bottoms of my breasts becoming slightly flatter, and I could feel the edge of my left implant near the inner end of my IMF incision/cleavage. I moved up my appointment, and my surgeon diagnosed capsular contracture based on physical examination (no imaging). He explained he thinks the scar tissue is pushing the implants up and in, which is why I am able to feel the edge of my implant. He prescribed montelukast 10mg daily for 3 months, plus daily massage, in hopes of slowing or stopping progression. He acknowledged another surgery might be necessary. When we asked about costs, he said we could file an Allergan warranty claim and basically said that his office would cover the surgical costs again. But since I wasn't in pain and still liked my appearance, we agreed to reassess in 3 months.
Oct. 2026 – Current situation
After approx. 2.5 months on montelukast, my breasts still look good. The slight flattening hasn't visibly worsened, and there's no obvious rippling. However, I can now easily feel BOTH implants near my inner cleavage, whereas initially it was only one and I can feel more on the one that was originally affected.
In the last few days, I've developed new tenderness/pain in my right breast, just inward from the nipple. At first I didn’t think much of it because my breasts get painful/tender around ovulation and in late luteal phase. But it is starting to feel very localized, which concerns me.
My follow-up with my surgeon is scheduled for Thanksgiving week (delayed slightly beyond 3 mos. due to travel), and I'm debating whether to call back to try and move it up. I’m also planning to request my complete medical records, but I'm concerned this might make the office defensive or jeopardize their willingness to cover another revision, because they will understand I am requesting this for either a lawyer, another doctor, or both.
My dilemma
I've spent thousands of dollars and most of 2026 recovering from surgeries or dealing with complications. Both recoveries significantly disrupted my work and personal life. I'm questioning now whether I trust my surgeon. He's board-certified, popular locally, has excellent before and after photos, good reviews and came highly recommended from people in my circle. As I mentioned I also previously worked in a plastic surgery office, so I felt reasonably informed when choosing him. He's been responsive and financially accommodating, but I can't help wondering whether these are unfortunate complications or signs of poor surgical judgment.
I've scheduled second-opinion consultations, but they're not until early 2027, and I've been quoted $9,000–$17,000 for another revision over the phone.
The frustrating part is that right now I STILL AM OKAY WITH HOW MY BREASTS LOOK. If appearance were the only consideration, as minimal as the effect is, I absolutely would not have another surgery. (As long as it stayed that way). I want to keep implants long-term, but I'm concerned about worsening contracture, pain, and what happens if I wait.
These seem to be my options:
Explant: I really don't want this. The idea of 3 surgeries in a year, only to end up with a worse result than what I started with, especially after getting used to my appearance with implants, feels devastating.
Leave it alone: If the appearance stays acceptable and pain is manageable, can I safely wait? Could I lose eligibility for the Allergan warranty or my surgeon's offer to cover a future surgery?
Revise with the same surgeon: Potentially little or no additional cost, but can I trust him? Am I at higher risk for another complication because of his technique, or could these genuinely be fluke complications?
Revise with another surgeon (and potentially pursuing this legally): Potentially $9-17k out of pocket. Is it reasonable to wait months for another opinion now that I'm experiencing pain? Would pursuing legal advice about recouping some of my expenses be worthwhile, even though I signed all the consent forms? How do I know who to trust, given that I went with a board-certified well known doctor initially and this still happened?
My biggest questions:
Has anyone experienced capsular contracture this early after a revision, particularly with GalaFLEX? What happened?
Would you trust the same surgeon for a third procedure, given these circumstances?
Is there a downside to waiting if my breasts still look good and the pain doesn't worsen?
Should I be asking for imaging, a specific Baker grade, or other testing?
What questions should I ask at my upcoming appointment?
Has anyone navigated Allergan warranty claims or a surgeon offering to cover multiple revisions?
I live about an hour from my surgeon, and another surgery would be a significant disruption, even if financially covered. Another surgery if not covered, especially given that it’s more money for a revision, would put me in a really bad position financially right now so soon after the first. What would you do if you were me? Am I overlooking anything? I'd really appreciate experiences or advice, especially from a surgeon, or anyone who's dealt with repeated complications but was otherwise happy with their results.
