Story Time Important tool to find doctors in the USA

This is from an old post about MAOI-friendly doctors I came across. Note that the US govt. website featured here can be slow, finicky, vexing, and hard to understand-- much like actual Americans! I kid, I kid... I'll try to give very detailed instructions. A couple more important notes:

Most search results for selegiline will be for neurologists treating parkinson's. There is a filter to view the doctor's specialty so you can see if they are a family doctor, psychiatrist, or neurologist. Check this so you don't waste time contacting a doctor who can't help you.

When adding generic drug names it's best to use the 'contains' search option rather than 'equals' because some drugs are listed with other words such as 'sulfate' or 'HCl' and may not show in the search results if, for example, you search 'equals' 'phenelzine'. This tool can also be used to find a prescriber in any city or state who has prescribed any particular drug of interest, not just mental health medications, so it can be useful for finding a doctor who has experience with uncommon or off-label prescribing for various medical conditions.

Okay, here's the link and instructions, and if anyone has better methods please share!

MAOI Dr. Search (US)

  • Upper left, select year. Yes, you have to do this for each search. Why, I don't know.
  • Click 'filter', then switch from 'basic filtering' to 'advanced filtering'.
  • In the 'select column' field choose 'Brnd_Name', change the 'condition' field to 'equals', and then in the 'enter value' field type the brand name you want (Parnate, Nardil, Emsam, Marplan, but only enter one). Spelling matters.
  • Click 'add filter condition', change 'and' to 'or'. Moving from left to right, this time select 'Gnrc_Name', 'contains', and type the generic name of the drug from the previous step (tranylcypromine, phenelzine, selegiline, isocarboxazid). Spelling matters.
  • If you want to search for more than just one MAOI, continue with 'add filter condition', always using 'or', using 'equals' for brand names, and using 'contains' for generic names.
  • When finished you may have anywhere from 2 to 8 entries. Look below for 'add filter group'. Change it to 'and'. Under 'select column' choose 'Prscrbr_State_Abrvtn' to search doctors in your state, or choose 'Prscrbr_City' to search only doctors in your city. In the next two fields select 'equals' and type your two letter state postal code (e.g. NY for New York, or GA for Georgia) or full city name. I suppose you could repeat this step as many times as desired, this time changing 'and' to 'or' and thereby also search for doctors in neighboring states or cities. I haven't tested that.
  • Hit the blue 'apply filters' button and get busy contacting!

This searches Medicare claims for these drugs from recent years. I've tried finding the same tool for Medicaid, which is probably more relevant for many of us, but can't locate that data. If you find it or discover any errors in the instructions, please let me know. Good luck!

Story Time PSA: it is possible to legally get access to MAOIs even if they are not registered in your country + short tutorial how to

hey,

so I come from Finland where we dont officially have Tranylcypromine and Phenelzine registered, at first, I thought there was no legal way for me to obtain these medications.

[2025 Edit: update: For the Finnish people reading this, the Finnish medicine agency Fimea recently approved Abbonate (Tranylcypromine) but I am not quite sure when it will be purchasable at the drugstore, if ever. if you want to try Tranylcypromine and live in Finland, the pharmacy subcontractor Oriola Oy sells the German Tranylcypromine brand called Jatrosom which is almost 40% cheaper than Glenmark Tranylcypromine, but you obviously need a special permit + prescription for it.]

I just picked up Tranylcypromine from the pharmacy and I will give a short tutorial how to.

I contacted a private psychiatrist (preferably PhD, ex-head psychiatrist or someone with much experience), and he contacted the national drug administration Fimea and got an approval from them, and then got an approval from the drugstore that is closest to where I live to import it as well as the prescription itself.

I leave a quote from Fimea, and Im fairly sure most other drug administration agencies will have a similar policy.

The use of medicinal products that have been granted a marketing authorization is always the primary option in patients' pharmacotherapy. However, in individual cases and for special therapeutic reasons, Fimea may authorise the release for consumption of a medicinal product that has no marketing authorization in Finland. A special permit is granted for up to one year based on a situation and case-specific overall assessment.

https://fimea.fi/en/pharmacies/special_permits

so if you dont know where to start, send an email to your national drug administration, I think later finding a psychiatrist willing to prescribe it will be the hard part.

having health insurance that covers private doctor meetings, as well as medication costs is also a good idea (if youre not sure, email your insurance company and ask if they cover it!).

the total price I paid for 28x10mg Glenmark Tranylcypromine was 106€

(+40€ single time Fimea fee)

in countries that doesnt have MAOIs officially registered, I highly recommend reading up a lot on the MAOI diet as they might have lacking experience with MAOIs, learning other info is also good, like its probably dangerous to combine them with SSRIs, if you get anesthesia eg. at the dentist or for a surgery, some forms of anesthesia can be dangerous. it also seems like its possible to combine MAOIs with stimulants.

you should also get a blood pressure monitor and monitor it some days before starting and some after, its a good idea at least to mention these topics to your psychiatrist so he/she can read up on them in case he/she doesnt know.

ofc the doctor should know how to use MAOIs, but you can never be too safe, especially cause misuse of MAOIs can result in serotonin syndrome and in the worst case death.

you can find loads of info about MAOIs here https://www.psychotropical.com/

and long MAOI diet [PDF] here https://www.psychotropical.com/wp-content/uploads/4.20-MAOI_diet_long.pdf

Parnate (Tranylcypromine) Parnate and nicotine interact

• Upvotes

I‘m sure a lot of people know this but I got to find out the hard way today. Nausea like crazy for a couple hours. It inhibits nicotine breakdown so your plasma levels increase and stay up longer, plus obviously more noradrenaline, adrenaline, and dopamine are released.

Used to be a smoker a few years ago (pack in a sitting sometimes), now 3 slims knocked me out.

How do the different neurotransmitters "feel"?

• Upvotes

I read people commenting about how they can "feel" serotonin when on SSRIs, as opposed to MAOIs, but no elaboration on what that feeling is.

I realise everyone is different and every experience will be different.

Can anyone explain to me how each of the neurotransmitters "feel" to them?

Is the feeling consistent across diagnoses?

In particular, I'm interested in differentiating between atypical depression with anxiety versus ADHD.

Nardil (Phenelzine) Nardil causing extra anxiety

Hi I have been taking Nardil for just over a month (9 days on 45mg)

I am taking it for Social Anxiety and have yet to experience it's anti-anxiety effects

The last 5 days I have felt extra anxious even just when at home where social anxiety is not an issue because I am in my safe space, but the last 5 days even doing simple/basic things like taking the rubbish out, checking the mail box has me feeling extra anxious before doing these things (anticipatory anxiety), looking out the windows and making sure neighbours aren't outside so they don't see me etc.. I am even experiencing GAD, something that has never been an issue for me, only Social Anxiety. is this normal or should I be concerned?

Parnate (Tranylcypromine) One year of parnate 🥹🫶🏻

A year ago I decided to try parnate for severe treatment resistant postnatal depression. I could cry thinking about how much it has helped me and wanted to share my story for others thinking of trying it (of course, YMMV). My depression was so severe I slept 14 hours, couldn’t even enjoy delicious food or a good tv show, cried daily for hours, and had constant thoughts that my new baby was better with a different mum. I did ECT, TMS, and tried countless medications. I was scared to try an MAOI for some reason and it took a few weeks of dizziness, nausea and random symptoms but after a few months I started enjoying things for the first time since my son was born. I could watch a tv show, sing, enjoy food. It brought back colour to my life. I was picturing my life being a dark hole forever and my son never knowing his fun weird silly mum. A year of parnate and I now sleep 9-10hrs, love going out for dinners again, sing and dance daily with my toddler, have been doing self care and made some mum friends. I was even singing alone in the car today enjoying the warm weather with the windows down. I never thought I’d find my way out but I did. Thank you parnate!

Parnate (Tranylcypromine) Can’t take standing blood pressure

I’ve tried every technique I read online and I just get an error message.

I’m worried my doctor won’t want to increase the dose of parnate without data on this.

Nardil (Phenelzine) Is the anti-anxiety feeling of Nardil similar to a benzo or more like propranolol? Or is it a "unique" feeling?

Have only been on Nardil for 35 days and 7 days on 45mg and have yet to experience it's anti-anxiety benefits.

Once it hits, what does it feel like? Is the feeling similar to a benzo or more like propranolol? Or is the anti-anxiety feeling on a class of its own?

I Need Advice please help me with what i’m experiencing

I got on 60mg of nardil after 3 days of 45mg. the first two weeks i felt really drunk , woozy, food tasted great, and i was giggling a lot like a drunk person .
week 3: fantastic mood and was able to walk through a store without anxiety. even though it didn’t touch a lot of my symptoms, these two parts of relief felt great. the best week i’ve had in years.
weeks 4-8: those benefits left. i decided to ride it out and hope maybe it’d come back but all i was left with was inorgasmia, constipation, and a bit of water weight gain
week 9: my doctor put me on 90mg. all i have felt was extremely tired. it’s been 4 days.

i don’t understand why the 60mg had me feeling something so briefly and then never worked. even if it didn’t touch 75% of my symptoms, the mood and ability to walk through a store without being anxious is what i miss. does this mean i shouldn’t even try 90mg since maybe the disappearance without reappearance of relief from 60mg is proof for some reason this medicine does not click within my body and mind? maybe because a history and dealing of neuroinflammation from lyme and mold and stuff? i just don’t get why week 3 happened and i got physical symptoms that have stayed if this medicine won’t work. i apologize for my ignorance. the science of MAOIS still confuse me!

I Need Advice Curiosity about diet

Years of agoraphobia and too many failed meds, I'm considering nardil.

The road block is that I'm autistic and have a very limited diet and foods that are safe for me. I eat a lot of chicken nuggets and potato type foods that are frozen. And I eat a ton of boxed white cheddar shells, Mac n cheese etc. burgers with cheese and what I believe are safe breakfast foods and fruit that are of no concern. Is boxed Mac cheese and Kraft slices safe? I eat them daily and would like opinions as the literature isnt consistent.

I know my diet is terrible and I'm working on it lol.

Appreciate anyone who has has taken the time to read this and or offer insight. Take care <3

Parnate help

Hi all, for those who have taken Parnate, what has your experience compared to nardil? I suffer from extreme anxiety and social anxiety, depression, but I also have ADHD. I’ve heard this is more stimulating than Nardil, which is why it interests me to see if it could help ADHD, focus, executive function more than Nardil did.

How to stop sugar cravings 😭

I’ll get up in the morning and feel like death until I have a bowl of oats drowned in maple syrup or sugar and constantly crave sugar throughout the day.

Carbs or increased appetite aren’t really a a problem so I haven’t actually gained weight I’m just eating so much sugar which is so unhealthy

I genuinely can’t stop it through restraint and I’m not keen on GLP1s would anything else help??

MAOI + stimulant Can you take stimulants everyday?

Is it okay to take stimulants everyday to help with intense drowsiness caused by MAOI's? Which stimulants are the best for this?

When to consider an MAOI

Been on an SSRI, 2 SNRIs, TCA, atypical. I don't really have anxiety or melancholy but prominent motivational apathy and anhedonia. Only drugs that briefly help my mood is caffeine and methylphenidate before crashing horribly. I can't try amphetamines or bupropion due to a seizure disorder, and I would like something that doesn't 'kick in' and 'come down' daily.

Would an MAOI (or anything else) provide a subtle but more sustained pro-motivational effect like a stimulant does?

Nardil (Phenelzine) Is the lowest effective dose for Social Anxiety 45mg or 60mg?

I am confused. For Social Anxiety some places online say 45mg, others say 60mg

Have been on 45mg for 6 days (On Nardil for 34 days in total) and I have not seen any Social Anxiety symptoms reduction yet, only side effects E.g severe hypersomnia, increased appetite and weakness/fatigue and some insomnia

When will this SA Gold Standard medication start working?

Parnate (Tranylcypromine) Shit. I just took some Dayquil without reading the back of the package.

I have a really bad cough / cold and I took some Dayquil to try to feel better. It says you should absolutely never take it if you are taking a MAOI. I take 30mg of parnate.

Now online is telling me I will get a hypertensive crisis. I am on hold with poison control. I also take 150mg of Wellbutrin but have never had serotonin syndrome.

Nardil (Phenelzine) How long does Nardil take to genuinely start working?

I started over a month ago at 45mg, and then increased to 60mg a little over 2 weeks ago. Do I still need to wait longer to feel something at 60mg? Or do I just need to increase the dose if it’s not working yet?
