Story Time Important tool to find doctors in the USA

This is from an old post about MAOI-friendly doctors I came across. Note that the US govt. website featured here can be slow, finicky, vexing, and hard to understand-- much like actual Americans! I kid, I kid... I'll try to give very detailed instructions. A couple more important notes:

Most search results for selegiline will be for neurologists treating parkinson's. There is a filter to view the doctor's specialty so you can see if they are a family doctor, psychiatrist, or neurologist. Check this so you don't waste time contacting a doctor who can't help you.

When adding generic drug names it's best to use the 'contains' search option rather than 'equals' because some drugs are listed with other words such as 'sulfate' or 'HCl' and may not show in the search results if, for example, you search 'equals' 'phenelzine'. This tool can also be used to find a prescriber in any city or state who has prescribed any particular drug of interest, not just mental health medications, so it can be useful for finding a doctor who has experience with uncommon or off-label prescribing for various medical conditions.

Okay, here's the link and instructions, and if anyone has better methods please share!

MAOI Dr. Search (US)

  • Upper left, select year. Yes, you have to do this for each search. Why, I don't know.
  • Click 'filter', then switch from 'basic filtering' to 'advanced filtering'.
  • In the 'select column' field choose 'Brnd_Name', change the 'condition' field to 'equals', and then in the 'enter value' field type the brand name you want (Parnate, Nardil, Emsam, Marplan, but only enter one). Spelling matters.
  • Click 'add filter condition', change 'and' to 'or'. Moving from left to right, this time select 'Gnrc_Name', 'contains', and type the generic name of the drug from the previous step (tranylcypromine, phenelzine, selegiline, isocarboxazid). Spelling matters.
  • If you want to search for more than just one MAOI, continue with 'add filter condition', always using 'or', using 'equals' for brand names, and using 'contains' for generic names.
  • When finished you may have anywhere from 2 to 8 entries. Look below for 'add filter group'. Change it to 'and'. Under 'select column' choose 'Prscrbr_State_Abrvtn' to search doctors in your state, or choose 'Prscrbr_City' to search only doctors in your city. In the next two fields select 'equals' and type your two letter state postal code (e.g. NY for New York, or GA for Georgia) or full city name. I suppose you could repeat this step as many times as desired, this time changing 'and' to 'or' and thereby also search for doctors in neighboring states or cities. I haven't tested that.
  • Hit the blue 'apply filters' button and get busy contacting!

This searches Medicare claims for these drugs from recent years. I've tried finding the same tool for Medicaid, which is probably more relevant for many of us, but can't locate that data. If you find it or discover any errors in the instructions, please let me know. Good luck!

Story Time PSA: it is possible to legally get access to MAOIs even if they are not registered in your country + short tutorial how to

hey,

so I come from Finland where we dont officially have Tranylcypromine and Phenelzine registered, at first, I thought there was no legal way for me to obtain these medications.

[2025 Edit: update: For the Finnish people reading this, the Finnish medicine agency Fimea recently approved Abbonate (Tranylcypromine) but I am not quite sure when it will be purchasable at the drugstore, if ever. if you want to try Tranylcypromine and live in Finland, the pharmacy subcontractor Oriola Oy sells the German Tranylcypromine brand called Jatrosom which is almost 40% cheaper than Glenmark Tranylcypromine, but you obviously need a special permit + prescription for it.]

I just picked up Tranylcypromine from the pharmacy and I will give a short tutorial how to.

I contacted a private psychiatrist (preferably PhD, ex-head psychiatrist or someone with much experience), and he contacted the national drug administration Fimea and got an approval from them, and then got an approval from the drugstore that is closest to where I live to import it as well as the prescription itself.

I leave a quote from Fimea, and Im fairly sure most other drug administration agencies will have a similar policy.

The use of medicinal products that have been granted a marketing authorization is always the primary option in patients' pharmacotherapy. However, in individual cases and for special therapeutic reasons, Fimea may authorise the release for consumption of a medicinal product that has no marketing authorization in Finland. A special permit is granted for up to one year based on a situation and case-specific overall assessment.

https://fimea.fi/en/pharmacies/special_permits

so if you dont know where to start, send an email to your national drug administration, I think later finding a psychiatrist willing to prescribe it will be the hard part.

having health insurance that covers private doctor meetings, as well as medication costs is also a good idea (if youre not sure, email your insurance company and ask if they cover it!).

the total price I paid for 28x10mg Glenmark Tranylcypromine was 106€

(+40€ single time Fimea fee)

in countries that doesnt have MAOIs officially registered, I highly recommend reading up a lot on the MAOI diet as they might have lacking experience with MAOIs, learning other info is also good, like its probably dangerous to combine them with SSRIs, if you get anesthesia eg. at the dentist or for a surgery, some forms of anesthesia can be dangerous. it also seems like its possible to combine MAOIs with stimulants.

you should also get a blood pressure monitor and monitor it some days before starting and some after, its a good idea at least to mention these topics to your psychiatrist so he/she can read up on them in case he/she doesnt know.

ofc the doctor should know how to use MAOIs, but you can never be too safe, especially cause misuse of MAOIs can result in serotonin syndrome and in the worst case death.

you can find loads of info about MAOIs here https://www.psychotropical.com/

I Need Advice please help me with what i’m experiencing

I got on 60mg of nardil after 3 days of 45mg. the first two weeks i felt really drunk , woozy, food tasted great, and i was giggling a lot like a drunk person .
week 3: fantastic mood and was able to walk through a store without anxiety. even though it didn’t touch a lot of my symptoms, these two parts of relief felt great. the best week i’ve had in years.
weeks 4-8: those benefits left. i decided to ride it out and hope maybe it’d come back but all i was left with was inorgasmia, constipation, and a bit of water weight gain
week 9: my doctor put me on 90mg. all i have felt was extremely tired. it’s been 4 days.

i don’t understand why the 60mg had me feeling something so briefly and then never worked. even if it didn’t touch 75% of my symptoms, the mood and ability to walk through a store without being anxious is what i miss. does this mean i shouldn’t even try 90mg since maybe the disappearance without reappearance of relief from 60mg is proof for some reason this medicine does not click within my body and mind? maybe because a history and dealing of neuroinflammation from lyme and mold and stuff? i just don’t get why week 3 happened and i got physical symptoms that have stayed if this medicine won’t work. i apologize for my ignorance. the science of MAOIS still confuse me!

Parnate help

Hi all, for those who have taken Parnate, what has your experience compared to nardil? I suffer from extreme anxiety and social anxiety, depression, but I also have ADHD. I’ve heard this is more stimulating than Nardil, which is why it interests me to see if it could help ADHD, focus, executive function more than Nardil did.

I Need Advice Curiosity about diet

Years of agoraphobia and too many failed meds, I'm considering nardil.

The road block is that I'm autistic and have a very limited diet and foods that are safe for me. I eat a lot of chicken nuggets and potato type foods that are frozen. And I eat a ton of boxed white cheddar shells, Mac n cheese etc. burgers with cheese and what I believe are safe breakfast foods and fruit that are of no concern. Is boxed Mac cheese and Kraft slices safe? I eat them daily and would like opinions as the literature isnt consistent.

I know my diet is terrible and I'm working on it lol.

Appreciate anyone who has has taken the time to read this and or offer insight. Take care <3

I’ve heard some say it is slow process of working, but I’ve also seen others say when it comes you start feeling the effectiveness very suddenly.

How to stop sugar cravings 😭

I’ll get up in the morning and feel like death until I have a bowl of oats drowned in maple syrup or sugar and constantly crave sugar throughout the day.

Carbs or increased appetite aren’t really a a problem so I haven’t actually gained weight I’m just eating so much sugar which is so unhealthy

I genuinely can’t stop it through restraint and I’m not keen on GLP1s would anything else help??

MAOI + stimulant Can you take stimulants everyday?

Is it okay to take stimulants everyday to help with intense drowsiness caused by MAOI's? Which stimulants are the best for this?

When to consider an MAOI

Been on an SSRI, 2 SNRIs, TCA, atypical. I don't really have anxiety or melancholy but prominent motivational apathy and anhedonia. Only drugs that briefly help my mood is caffeine and methylphenidate before crashing horribly. I can't try amphetamines or bupropion due to a seizure disorder, and I would like something that doesn't 'kick in' and 'come down' daily.

Would an MAOI (or anything else) provide a subtle but more sustained pro-motivational effect like a stimulant does?

Nardil (Phenelzine) Is the lowest effective dose for Social Anxiety 45mg or 60mg?

I am confused. For Social Anxiety some places online say 45mg, others say 60mg

Have been on 45mg for 6 days (On Nardil for 34 days in total) and I have not seen any Social Anxiety symptoms reduction yet, only side effects E.g severe hypersomnia, increased appetite and weakness/fatigue and some insomnia

When will this SA Gold Standard medication start working?

Parnate (Tranylcypromine) Shit. I just took some Dayquil without reading the back of the package.

I have a really bad cough / cold and I took some Dayquil to try to feel better. It says you should absolutely never take it if you are taking a MAOI. I take 30mg of parnate.

Now online is telling me I will get a hypertensive crisis. I am on hold with poison control. I also take 150mg of Wellbutrin but have never had serotonin syndrome.

Nardil (Phenelzine) How long does Nardil take to genuinely start working?

I started over a month ago at 45mg, and then increased to 60mg a little over 2 weeks ago. Do I still need to wait longer to feel something at 60mg? Or do I just need to increase the dose if it’s not working yet?

Nardil (Phenelzine) Alternatives for nausea and MAOIs

Hey everyone, longtime Nardil patient here and I’ve ran into my first roadblock with MAOIs. I’m not finding anything that can be used for nausea, only no therapeutic doses of ginger (soda/tea). I’ve read that while Zofran may be an option, it is not fully understood whether ST can accumulate and cause ST syndrome. AI claims it is done so with antagonism of one ST receptor, forcing an accumulation of ST in the brain. Has anyone safely used an alternative medication for nausea while on a MAOI?

MAOI + stimulant i've started pirlindole again. my favorite antidepressant

had to quit it due to summer heat in august. now it's cold outside and depression creeps in

this drug has been my best friend last year, even with side effects. i can actually feel it helping me even tho it's subtle

and i would vote for Pirlindole post flair (it should be baby blue color. pills are just white)

i also take Noopept, Coq10, palmitoylethanolamide and a shit ton of caffeine. fell in love with Red Bulls recently (sorry body)

let's hope it will help 🤍

Nardil (Phenelzine) Weight loss - very slow?

We all know weight loss is hard on Nardil. Those who have succeeded, or at least lost a decent amount of weight. Did it just take a lot longer than normal and you had to grind? How much harder does Nardil make it? Surely eating in a decent deficit for long enough, you lose weight? Thanks to anyone who responds 😄

Parnate (Tranylcypromine) What does parnate feel like?

I’m asking my psychiatrist for it. He prescribed me Segelegine but my insurance didn’t cover it. I tried Wellbutrin, Zoloft, and many other ssris and snris and I’m 19. Zoloft helped some, but after a while I felt depressed again.

Wellbutrin just made me a really anxious. What do maois feel like? No antidepressant I’ve tried gave me any happiness or hope for the future. It’s more of a complete content and lazy feeling. No emotions or anything to look forward to. No motivation just flat.

I fast tapered off my Effexor in February (too fast) and I’ve been brain fried since then I can’t think, my depression has been getting worse slowly and my accutane gave me horrible exhaustion and I’ve been off it for 4

Parnate (Tranylcypromine) Currently on atomoxine and wanna get on parnate too, afraid psychiatrist won't allow me

Hey Ive been taking atomoxine for my ADHD and it's been helping but not really for mood and motivation, I bedrot all day watching anime and I'm thinking of adding parnate but not sure if psychiatrist would allow me. So would it be better to see another psychiatrist?

r/MAOIs • • 4d ago

Nardil (Phenelzine) Rot in bed all day watching anime. No motivation to do anything productive. Taking 225 effexor and nothing..

Ive been taking effexor since April for my social anxiety and it helped reduced my mental social anxiety a bit but I still stay in my bed all the time not sure why I thought It was ADHD so I complained about it to my psychiatrist and he prescribed atomoxine I'm like 3 weeks 40mg and 2 weeks and 3 days on 60mg (moved up) and it helped quiet my mind a bit more but I still feel the same I still don't have motivation to do activities and do my learning. I'm considering switching to nardil but I don't know if that'll help. Sometimes my mood goes up which makes me motivated to do stuff but it's pretty temporary and lasts like 1 minute before falling back

Parnate (Tranylcypromine) Is this what Trivon does to people?

I've been taking trivon (tranylcypromine from INTAS) for the past 2 months. On 10mg for the first 10 days and 20mg for the rest. This is the first time I've reacted this strongly to an antidepressant. After taking it in the morning I would feel incredibly tired for the rest of the day. Sometimes just standing up makes me see blotches and random color patches, like a dying CMOS on a camera struggling to produce a coherent image. So far what I've been experiencing has been the complete opposite of what I'm expecting. It's the opposite of stimulating, and it makes doing everyday tasks very hard.

Antidepressants generally don't give me much trouble. I've been on all 5 SSRIs and 2 SNRIs and quit all of them cold turkey without feeling much. This trivon stuff though is something else, I am getting more reaction out of this than the 7 SSRI/SNRIs I have taken combined. Right now I'm more inclined to believe it's the manufacturer rather than tranylcypromine cuz there is no way an antidepressant makes you feel like this.

Tips & Tricks why are there no mods here?

hello? alive?

if none of you are going to ever check in, make me a mod.

at least I exercise a modicum of attention to the people asking questions here.

edit: people keep citing vivid and Technical as if this was personal. it's not, I am acquainted with both of them and I like them and know they are way more knowledgeable than I am.

there are like three other mods that are never here.

don't make assumptions. and also relax.

and feel free to check my history if you're wondering how much I've contributed to this sub over the years. including generosity towards anyone I've responded to, whether they're facing a common issue that could easily be solved via a sub search, or an extremely challenging hurdle.

edit 2: also, keep downvoting me and don't say anything, because that's useful. if you leave even one downvote, show your cowardly face and go over what you've done for this subreddit.
