This patient’s main concerns were the narrow distance between the eyes and eyebrows, along with significant sagging around the outer corners of the eyes. The drooping tissue covered the outer portions of the double eyelid creases, making the eyes appear heavy and less open. Although the patient had previously undergone a forehead lift, the results had not met their expectations, making the decision to undergo revision surgery particularly difficult.
The revision forehead lift was a technically challenging procedure that required careful release of adhesions from the previous surgery, management of existing scar tissue, and precise repositioning of the drooping outer eyebrows and surrounding tissues. Special attention was given to maintaining a balanced distance between the eyes and eyebrows while allowing the outer portions of the double eyelid creases to become naturally more visible.
The patient was also concerned about the downward-slanting outer corners of the eyes, which contributed to a tired and aged appearance. Through Fox Eye Surgery (Lateral Canthoplasty, we enhanced the horizontal appearance of the eyes and gently elevated the outer corners, creating a longer, more refined, and naturally lifted fox-eye shape.)
To address the overall facial sagging, Thread Lifting was performed to lift and redefine the cheeks, areas around the mouth, and facial contours. Rather than focusing solely on the eyes, the combination of procedures helped create a more harmonious, refreshed, and youthful-looking appearance. The patient was especially pleased with how the overall facial contours appeared more lifted and defined.
Additionally, the patient had concerns about a long-looking philtrum and lips that appeared to turn inward. These concerns were addressed through Philtrum Reduction (Lip Lift and Philtral Dimple Creation, which helped enhance the natural contours and definition of the upper lip.)
The nasal alae were also relatively wide and drooping. By combining Alar Lift and Alar Reduction, we refined the nostril shape and created a more balanced and aesthetically pleasing alar-columellar relationship (ACR.)
Because the patient had been disappointed with the outcome of the previous forehead lift, they were understandably cautious and concerned about undergoing another procedure. However, following surgery, the patient expressed great satisfaction with the improved visibility of the outer double eyelid creases, the more naturally elevated outer eye corners, and the refined overall facial contours.
What made this case particularly memorable was the patient's happiness at seeing several long-standing concerns addressed together. From the eyes and eyebrows to the philtrum, lips, and nasal alae, each area was carefully refined to complement the patient's overall facial proportions.
The patient shared that, this time, they finally felt that the concerns they had hoped to improve were properly addressed. Seeing their happiness and renewed confidence made this case especially rewarding for our team.
Individual results may vary depending on each patient's anatomy, surgical history, and healing process.
