I was wondering how they missed it? Were the ultrasound tech bad? Woudnt they have done the full work up as well just cause he’s a high stake athlete (d dimer, CTPE, contrast venography, ect).

I just came up with a new game. For the uninitiated the aristocrats is an improv exercise for comedians where they make up the wildest grossest joke they can think of within the scenario of a family circus act auditioning to their agent. Sort of like a verbal meme template. This is my dumb medical version.

I open up Doximity scribe and I dictate the most insanely sick patient I can think of in under one minute. My last example: 120 year old female with 150 pack year history comes in with crushing chest pain and left sided deficits, multiple stab wounds and piece of rebar sticking out of her head. CT chest is revealing for severe fibrosis and saddle PE. WBCs are 80 and lactic acid 30. Cultures growing CRAB and MDRO everything. She is quadriplegic, blind and has active psychosis.

Then I generate the note and open up the “refine ask” feature and see if I can get it to immediately recommend goals of care discussion when I ask it for next steps. It is surprisingly hard because it often thinks this is a stupid board question. Also a part of me hopes that this will fuck up the AI data collection and keep the robots on their toes.

I failed the final written exam of my residency.. 5 years of brutal work, ended with failure. I'm lost.. i made plans for such scenario.. but the fact it happened made me uncertain, angry and jaded..

I am a junior resident (F) and I have a crush on one of the senior residents I work with (F), shes drop dead gorgeous so no wonder I'm crushing on her, but this resident is happily married to a man, and she's clearly straight af, I'm just venting because it just feels so high school to crush on someone at work lol, especially that the crush has gone for around a year now???? I do not plan on acting on it and honestly it doesn't effect my life much and I am keeping a strictly professional and friendly relationship with her, I just really wonder why do I still feel so attracted to her still -.-

Intern year feels bad man

Why is it that every ward floor in this hospital has like 15 empty rooms but only one working printer. And that printer is always on the other side of the planet. I just need one sheet for a script why is this my life.

How judicious should I be with iv fluid boluses for patients like this in this setting? Or maybe patients with ascites?

For context I’m a PGY-2 FM resident in an unopposed program. I love a lot of what I can offer patients and am happy with my training thus far. But the classic debate regarding pursuing fellowship to have a “niche” and have a more narrow scope keeps popping into my head. I can’t tell whether it’s because primary care is so broad and challenging vs me wanting more specific “expertise” in a field. I’m considering addiction medicine fellowship as I find it incredibly interesting and rewarding. Was curious if anyone else in FM/IM feel like this and any advice would be appreciated regarding specialization vs broad practice. Thanks!

Hey everyone, after 2 days I have to make a life changing decision, which is choosing my speciality for residency.

I really like ophthalmo, but I'm someone who has essential tremors and I'm afraid this could be a problem, especially when it comes to surgery. Sometimes my tremors are very bad , other times they're very slight.

My father also has these tremors, but his got much worse as he got older, and that's something that worries me too.

Is there anyone here with essential tremors who chose ophthalmo? Or anyone who knows someone in this situation? I'd really appreciate it if you could share your experience.

...you wish you could say to your residents, but can't?

Or generally tips that are more "real life" / experience-based tips that you have to hold back on?

For example, I had an experience recently where I (despite having pretty good instincts/street smarts) got taken a little bit for a ride by a possible (malingering? psychosomatic?) patient. Looking back my attending (& consultant) likely knew what was going on the whole time, but let me get in the weeds with it rather than grabbing me by the collar and laying down the situation like I wish they had - perhaps because they didn't want to bias me or crush my spirit or something, but I'm not sure. I have a couple docs that will speak freely and I really wish they all would, because I'm not great at reading between lines.

So, what are some things you have held back on, but wish you could say to residents?

Question to IM residents all PGYs

I’m considering IM residency bc I’m very interested in this field, but I’m very very scared about the 60-80h per week and the busyness of this specialty. How do you survive all that? I’m not a slacker, I like working and being busy but that many hours and all the stress with super sick patients make me scared and wondering if it’ll overwhelm me.
Is this the work schedule during the entire residency? And what about after residency? Is it going to calm down if staying in a hospital setting?
How do you guys handle all this?
Any advice?

I’m 30 years old and a pathology resident. I’m currently preparing a presentation on two projects I’ve worked on to present to my department and I’m completely stressed out.
I honestly don’t have the time or energy for this. I feel like I’m going to be torn apart tomorrow with endless questions and drama…
In moments like these, I feel like I’m back in 9th grade, terrified that my teachers are going to scold me and punish me.
I’m a grown woman. I just want a little peace in my life, especially when I already have so much on my plate.
It’s absolutely exhausting. And ridiculous.

I am a fresh IM doc and so far I love IM, except the part that we get to engage with literally all types of bodily fluids of the patients on a daily basis. Seeing fecal residues left on the intestinal folds during a colonoscopy gives me nausea (I mean it), seeing feces on my double gloved finger after a rectal exam is even worse. Abscesses, diabetic feet, fourniers, patients with poor hygiene and the list goes on... I understand that each of these secretions are biological and we were trained to help poor patients regardless of how bad it looks/smells. I just want to come into terms with dealing with such stuff. I appreciate any recommendation 😔

One of attendings has a signature stamp that he gives to whoever is consenting the patient in preop so he doesn't have to walk by and say hi to the patient. He has a signet ring that he used to use (and still wears), but once Epic came around and everything needed to be scanned in they said that the sealing wax would mess up the scanner and told him he couldn't use it anymore. He threatened to quit and continued to do it until the department made him a stamp with the same design as the ring.

Anyway everyday before rounds he makes the resident team queue and kiss the ring. Everyone fights to be first so they aren't effectively making out with each other as he goes down the line. I, however, always take the last spot and make sure to kiss with tongue. He really appreciates that. Rumor has it at the end of the rotation he takes his favorite resident to his back office and drizzles hot wax on them and uses his ring......

Does anyone feel that way? I am an intern. I have VERY little time for self-learning and reflection. My program is workaholic, and most of my time is spent on non-educational tasks. The very little time I get to think and reflect on medicine, I have to study for Step 3 because I need to take it before the intern year ends per program policy. Does anyone else feel this exam actively jeopardizes resident education?

Hello, i have a question.
I have an opportunity or offer from two places for a residency. First one, big state hospital with smaller pay where i can learn a lot, second one is from a smaller private hospital with more pay(300 euro more) where i can learn less than the state hospital, but it is less stressful due to less surgical departments. What do you think?
What is more important at the beginning of the residency? To learn more or to earn more?

I’d never rx or even inbasket after after even one drink, but I guess that’s the difference between graduate level education dedicated towards medicind and a degree earned online while working 3 12s a week. That previously required YEARS of nursing experience to even be a candidate for, (The reason I’m so confident is my GME faculty literally doesn’t bother charting patients when I’m on inpatient as a senior and told me so as a monolith in ccc)

Edit: to be fair I don’t hate midlevels, however most aren’t trained at all.

A well trained midlevel should support the physician in their specialty they are working with and EXTEND THE CARE PRESENCE, not be like haha I can prescribe but I don’t know the difference between selective and non selective beta blockade.
