medications Biologics keep losing effectiveness/ Long term options

Has anyone else had several biologics lose effectiveness over time?

I was on Stelara for 6 years before it stopped working, even after doubling the dose in the last months before stopping. I then switched to Bimzelx, which I had to stop after a year due to bad side effects (infections etc.) and loss of effectiveness. Now, after about a year on Skyrizi, I'm experiencing constant painful flare-ups again.
Before biologics, I tried MTX and topicals (but those aren't viable options anymore for me)

I'm getting really stressed about what comes next, especially since I've already failed multiple biologics. I'm 21, and the thought of potentially going through this repeatedly for the rest of my life is pretty overwhelming.

Has anyone been in a similar situation? What are you on now, and did you eventually find something that worked long-term?

general How to remove the flakes from the scalp, so that I can use the medication to penetrate properly?

• Upvotes

I know we shouldn't remove the flakes for the Koebner phenomenon.

At the same time, the topical like ketoconazole and even clobetasol won't penetrate properly if you have a thick flake preventing it.

So what is the best way to remove the flakes? Can oils be used? Or anything?

phototherapy UV Phototherapy hasn’t worked - what now? (UK)

Just coming to the end of my phototherapy course (NHS) and in my final skin assessment today, I was told that it basically hasn’t worked and that I’m going to have to speak to a consultant within the next few months about “other options”, and have been told to go back on enstilar (steroid cream) until then.

Just curious about those who have been in this situation, what came next for you? What did the consultant suggest next?
