STICKY: SIBO Summary - Symptoms, Diagnosis, Treatment

Below please find a living document that summarizes the key information around Small Intestinal Bacterial Overgrowth ("SIBO"). Please comment with any additional information or research for inclusion consideration. Version 1.0 is summary material; I will be adding more details and citations for specific studies.

SIBO, as the name implies, occurs when bacteria overgrow the small intestine. The small intestine should have a low concentration of bacteria due to the presence of stomach acids and peristalsis, the wave-like muscle movement in the intestines. For context, stomach and proximal small intestine would typically have about 103/mL of bacteria, while the terminal ileum (end of the small bowel as it gets close to the colon) about 109/mL (or 1,000,000 times more), and the colon about 1012/mL (or 1,000,000,000 times more).

Symptoms

The overgrowth of this bacteria will present with a number of symptoms:

  • Bloating after eating ("postprandial") - most common symptom
  • Flatulence, often malodorous
  • Loose, watery stools (more common in Hydrogen-dominant SIBO)
  • Constipation (more common in Methane-dominant SIBO)
  • Absorption problems
    • Weight loss / inability to gain weight
    • Fat and fat-soluble vitamin deficiencies, particularly Vitamins A, D, and K
    • Floating stools (from fat malabsorption)
    • Vitamin B12 malabsorpiton
    • Protein and Carbohydrate malabsorption
  • Systemic problems
    • Overgrowth of bacteria in the small intestine can increase production of toxins and intestinal permeability
    • This has been less studied, but less serious effects include:
      • brain fog
      • confusion
      • anxiety
      • depression
    • More serious complications can include
      • hepatic encephalopathy
      • D-lactic acidosis
      • nonalcoholic fatty liver disease
    • Various conditions have increased correlations, including
      • Rosacea
      • Eczema
      • Food intolerances

Diagnosis

I will split this section into practical steps and clinical diagnosis.

Practically, a gastroenterologist will typically rule out other conditions first:

  • Physical exam
  • Colonoscopy and Endoscopy
  • Abdomen ultrasound
  • Stool test for parasites

At that time, if your symptoms match SIBO, your doctor may go directly to treatment. But otherwise these are the clinical tests:

BREATH TEST

This is the most common diagnostic method due to its low cost and limited invasiveness. Unfortunately, studies have been mixed on the sensitivity and specificity, with ranges between 30% and 75% -- hence why some doctors skip the test and go directly to treatment.

There are a number of preparations:

  • Antibiotics avoided for four weeks prior
  • Prokinetic drugs and laxatives avoided for one week prior
  • Complex carbs avoided for 12 hours prior
  • Exercise and smoking avoided day-of

For the actual test, you'll measure hydrogen and methane levels at baseline. Then drink either 10g lactulose or 75g glucose with one cup of water. Then your breath is measured every 15 minutes for 120 minutes.

There's some art to identifying a positive test; one semi-official criteria is:

  • methane level of >= 10ppm at any time during the test; or
  • hydrogen that increases >= 20ppm above the baseline level

Recently, new research has been investigating another typo of SIBO, that's dominated by Hydrogen Sulfide. Unfortunately, traditional breath tests cannot identify this gas, and someone with "flat-line" Hydrogen and Methane symptoms could be suffering from Hydrogen Sulfide SIBO. This version is typically characterized by "rotten egg" smelling gas, and may be worsened by eating high sulfur foods.

CULTURE

Historically a jejunal aspirate was done and concentration of bacterial colonies were measured, with an elevated level of > 103/mL being positive for SIBO. There are a number of issues with this:

  • overgrowth may be patchy, and a single sample may miss it
  • not all SIBO bacteria can be cultured/identified
  • samples can be contaminated during/after sampling

Treatment

Antibiotics

The current best practice prescription treatment is:

  • Hydrogen-dominant: Xifaxan, typically 550mg x 3 times daily, for 10-14 days. Studies have shown Xifaxan alone can be 50-65% effective, but Xifaxan + 5g daily of Partially Hydrolyzed Guar Gum can be 80%+ effective.
  • Methane-dominant: Xifaxan (550mg x 3 daily) plus Neomycin (500mg x 2 daily) for 10-14 days. The use of PHGG for methane-dominant has not been evaluated, but it's likely to be beneficial.

Mod's note-- personally, if your doctor is onboard, I think dosing with Xifaxan + Neomycin + PHGG is the best way to "cover your bases". The best place to find PHGG: https://sunfiber.com/products/

Important: because these antibiotics only operate selectively in the GI tract, and are NOT absorbed by the body, they are unlikely to cause the systemic issues associated with antibiotic use, making them safer. Additionally, Xifaxan crystallizes before it gets to the large intestine, meaning it should not affect the all-important microbiome.

Herbal Therapy

Additionally, studies have shown similar levels of success with over-the-counter "herbal" treatments. Two options; I believe each are two capsules twice daily for four weeks, but please confirm:

  • Dysbiocide and FC Cidal (Biotics Research Laboratories, Rosenberg, Texas)
  • Candibactin-AR and Candibactin-BR (Metagenics, Inc, Aliso Viejo, California)

Remission

Unfortunately, SIBO has very high rates of recurrence. Some possible ways to reduce recurrence chances:

  • Switch to a low FODMAP diet for 6 weeks after treatment, to starve any remaining bacteria and prevent regrowth
  • Incorporate a prokinetic, such as low dose Naltroxene, erithromycin, or even over-the-counter products such as Iberogast

Many people can avoid symptoms of their SIBO by switching to special diets, sometimes very restrictive ones. This is not a cure, but simply symptom management. A true cure addresses the underlying cause of the SIBO, and lets the patient eat "normally" without any effects (short of unrelated intolerances).

Hopefully this helps people, and I look forward to updating this and cleaning it up over time!

Moderation statement on accessibility.

This subreddit exists to support people with disabilities. Accessibility is the primary concern always.

Recently there has been an increase in report-spamming and downvoting of posts described as repetitive, particularly questions that have been asked before. This behavior is not acceptable in this community.

Many disabled users cannot easily navigate Reddit’s search function. Many are new, exhausted, overwhelmed, or asking questions while in distress. Repetition is not a failure of the user. It is a reality of accessibility.

Downvoting questions actively harms visibility and silences people who are already navigating barriers. Using the report function as a way to express annoyance or enforce personal preferences inappropriate. Reports are for rule violations.

If you encounter a question you believe has already been answered you have three options.

You can answer it again. You can link to an existing thread or resource. Or you can scroll past it. If you have the time to downvote or submit a report, you have the time to paste a link.

Anything else is consciously opting for exclusion. Personal attacks on users and the mod team will not be tolerated.

This moderation team will not prioritize the desire for a tightly curated, repetition-free feed over the needs of disabled people seeking help. We will not discourage questions in the name of efficiency, aesthetics, or personal frustration.

If this approach does not work for you, you are free to mute the subreddit, leave it, or create a different space with different rules. What will not happen is this community shifting away from accessibility-first moderation.

Misuse of the report function has already been passed onto the reddit moderators in a number of cases and will continue with a much broader brush after issuing this warning.

Thank you to those who respond with patience, share resources, or simply allow people to ask what they need to ask. That is what keeps this space usable.

I understand this condition is frustrating but this community will not turn into a circular firing squad. To target this frustration towards yourself or fellow members of the disability community is to serve those who stand to profit from illness.

This space exists to reduce harm not redirect it sideways.

Mixed SIBO, and now a possible underlying condition identified.

For many years I used antibiotics and antimicrobials to get temporarily reliefs. Symptoms always came back.

I tested positive for Mixed SIBO in the second half of the small intestine.

3 years ago, MRCPs revealed a stable bile duct stricture. As all liver values are normal, I never got the ERCP.

Recently, a gastroenterologist ordered a capsule endoscopy. You ingest a capsule that takes thousand of pictures of the small intestine. Result... Crohn in the second half of the small intestine, exactly where the Mixed SIBO is sitting.

Crohn is often an underlying cause of SIBO.

This Crohn might also be causing the bile duct stricture.

It took me 13 years, more than 70 doctors or other medically related professions, and an immense distress.

3 day course of rifaximin?

Has anyone found success with shorter courses of Rifaximin, e.g 3 days?

Backstory: I've had a previous positive SIBO result but past 2 tests were negative (including a negative Triosmart).

Never took Rifaximin although I was offered a 2 week treatment. But I'm still sensitive to fodmaps (bloating + diarrhea, and even fiber triggers diarrhea), and stool test results find overgrowth bacteria.

I'm hesitant to take rifaximin because some people have reported taking it, getting worse, and have never recovered. Thinking of a compromise where I take it for 3 days.

Symptoms Advice/Symptoms- Trio smart breath test

Symptoms:

Persistent mild upper ab discomfort.
Lower ab pain through the night ~ 7 hours after eating.
Lots of burping.
Slightly elevated amylase/lipase.

Clean endoscopy x2, colonoscopy, MRCP.

Stool test showed no bifido or lactobacillus.

Is a SIBO test the next obvious step? Doctors are baffled. Also in waiting for an EUS.

Dr. Pimentel Q&A 10/14/2026

Thumbnail
youtube.com
42 Upvotes

Dr. Pimentel did a Q&A this week. A list of the question topics is in the description of the video. They are listed in order, so it should be pretty easy to skip to the topics you care about.

You can find a full transcript here: https://www.reddit.com/r/SIBO/s/0Y3NwMb3GI

Points that stood out to me:

  • Dr. Pimentel once again emphasized that hydrogen sulfide is the most aggravating of the three gases, and a two-gas breath test misses it. If you are having diarrhea and urgency, those are signs that you should test for H2S using the three-gas breath test (Trio-Smart). If you're going to pay for a test, it might as well come with all three gases.
  • He announced for the first time that their lab has improved upon their CS-06 molecule, developing CS-06A. It is more effective than CS-06, reducing methane by 90%. This puts them on track for a human trial in late 2027. This is great news for IMO patients!
  • He mentioned that testing substrate (glucose vs lactulose) doesn't matter for methane or hydrogen sulfide, but glucose is more likely to miss hydrogen. I interpret this as you probably don't even need a substrate to detect methane or hydrogen sulfide. The main thing he emphasizes regarding getting an accurate breath test result is to not take prokinetics or laxatives in the days leading up to the test as you don't want to do the test right after you've temporarily flushed out a lot of microbes.
  • For some reason, patients with SIBO/IBS tend to have less polyps and less colon cancer. It could be that the autoimmunity driving SIBO/IBS or the changes in the microbiome are protective against cancer. They do not know the cause, but it is an interesting finding.
  • "How do you find the root cause of SIBO?"
    • Start with the antibody test (IBS-Smart). That tells you if it was from food poisoning. I rarely see IBS-Smart results on this subreddit. I think more people need to know about this.
    • If it's negative, you will have to think about things that may obstruct the bowels or slow gut motility. An example that he gave was that he asked a patient who had been suffering with SIBO for many years if she can remember if there was anything that occurred before the onset of SIBO. All she could remember was that her bowels were never the same after her appendix removal. So they did a barium study and the radiologist found an adhesion that caused the small bowel to narrow.
  • Bacteria do not build resistance to rifaximin. You can do repeated courses of rifaximin and it will continue to work, unlike other antibiotics. They did a study that shows this.
  • MAST is working very hard to find a treatment for anti-vinculin, but it takes a lot of money and resources. They get some grant money and other sources of funding and try to keep the ball rolling. My note: Donate to the MAST program to support their research! The more money they get, the faster they can push research and development to bring us a cure. Pimentel tries not to beg the public for donations since he thinks the government and other funding allocators should be giving a lot more money towards IBS/SIBO research as they do for other diseases. But I have no shame in plugging their fundraiser: https://support.cedars-sinai.edu/fundraiser/2127909
  • There is no relationship between PPI usage and SIBO based on the contemporary evidence.
  • Herbal antimicrobials (oregano oil, berberine, etc.) can be effective, but be careful taking them and take them under supervision. For example, too much berberine can be bad for your liver. Additionally, microbes can develop resistance to herbal antimicrobials. He has mentioned in the past that allicin works on IMO, but it loses its effect over time.
  • He tests for fungal overgrowth if the patient experiences bloating symptoms despite negative breath test. They do this by taking a sample with an upper endoscopy and sending it to a lab to test for fungal culture. He thinks that out of 100 overgrowth patients, less than 5 would have fungal overgrowth. It is a thing, but uncommon.
  • There is a new book on the way. Chapter 1 just got finished. Expected to be done in 8 months.
  • Generic rifaximin could come out in 2027, but for sure in 2028. Exciting for US-based patients. New-generation rifaximin is in development and there is an ongoing study, hopefully completed next summer. I made a post about this study a while back if you are in the LA area and are interested in participating: https://www.reddit.com/r/SIBO/comments/1u3fvkh/clinical_trial_for_new_therapy_for_ibsd_sibo_in/. Direct link to get in touch with them about this study: https://iredcap.csmc.edu/surveys/?s=TD7CHK3DC3

There are plenty of other very interesting questions, but some were too specific or basic for me to note down, so I encourage you to skip through the video yourself!

1st Time Treating SIBO --Is Relapse Prevention Necessary Yet?

Hi everybody,

So I recently tested positive for SIBO (for the first time) and am now coming to the end of my course of Rifaximin. I understand from reading other people's posts that there's a whole bunch of techniques and routines that people follow for preventing relapse, but I wanted to ask if this was necessary for the first experience with SIBO?

As far as I'm aware, I don't believe that I have a motility problem of some sort (though I might, I just simply don't know), so would it be more helpful to just wait and see if if it returns so that I'm not taking medication unnecessarily? I also feel that if I wait and do nothing, and then it does return, then I can confirm that I have a motility problem instead of just not knowing. What do people think?

I haven't been able to find a lot of advice online about what to do after having SIBO for the very first time, and I don't have my follow-up with my GI for quite some time. Should I try to get another breath test as well, to make sure that it's gone? Any and all advice would be very welcome!

Symptoms Help! No clue if I have SIBO or a weird stomach bug

So about 2 weeks ago, I randomly had intense body aches and heart burn for 2ish days. Took a covid and flu test and it was negative. I never had any diarrhea (except the one day I drank), but I’m definitely a bit constipated and my BM haven’t been great since this started. I also have eczema, acne, loss of appetite, and brain fog issues as well.

Nothing is super severe, but I am concerned cause it’s been happening for so long. So I wanted to ask someone if this is similar to their experience before they were diagnosed??

Treatments Officially diagnosed… now what?

Hello all~

I recently was diagnosed with hydrogen SIBO and let me tell you it’s the most frustrating health issue I’ve ever had to deal with. I miss work often because I cannot leave the restroom or the bloating is so painful.

My dr has already told me that they are recommending Rifaximin, but we have yet to discuss diet, supplements, etc. Does anyone have personal experiences with Rifaximin or other things they have done that have helped hydrogen SIBO? I asked my pharmacy and they pretty much freaked me out and said they cannot fill a prescription for Rifaximin bc it can cause brain bleeds. No thanks!

Any advice helps!!

I'm on the entry-level sertraline dose of 50mg, is it okay to combine with oil of oregano and allicin? I have used AI and it's mostly okay but apparently can be some blood thinning concerns. Just looking for good takes as I have SIBO.

H2S SIBO and constipation?

Diagnosed via breath test with H2S SIBO. Main symptoms: constipation, bloating. Comprehensive stool test showed high fecal fat but normal fecal elastase. CT scans, colonoscopy, ultrasounds showed no abnormalities.

Is anyone else with Sibo (h2s) is having same symptoms? According to Dr. Pemintel h2s Sibo is normally linked with diarrhea and not with slow motility.

Symptoms The hottest, stinkiest farts ever?

I’ve experienced IBS symptoms my entire life, but nothing like this. For the past two ish weeks, I’ve had hot, sulfuric farts. The stinkiest I’ve ever experienced. And they’re persistent, too. I can’t go more than five minutes without farting. I have increased my protein intake, so I think that might be to blame. But I’m not sure what to do. Does this sound like SIBO?

ICEOVAL VALVE Pelvic floor dysfunction and Sibo

I am pretty sure I found my root cause. Years before being diagnosed with sibo I would get sharp, sudden, intense, debilitating stabbing pains in my lower abdomen that were intermittent and short lived. Six months ago, I went to Er because it felt like my appendix was going to explode, but CT scan was clear. Fast forward to today, I had an ultra sound for Pelvic floor dysfunction yesterday because I told my GI dr that I was sitting on the toilet for an hour every morning trying to go. She said "ugh, thats definitley not normal!" The ultrasound revealed that my muscles were tight and not normal. So, i began therapy yesterday and started researching more on PFD. I came across ICEOVAL VALVE syndrome which apparently causes all the sharp excruciating symptoms I have been having all these years. When the valve is stuck open it allows bacteria to flow through which causes guess what, MOTHA FRICKING SIBO!!!! All of this I belive was caused by self destructing Binge drinking. It directly effects the valve.

Day 14 no booze Thoughts welcomed!!!

Treatments Finally got approved for Xifaxan

Finally picked up the Xifaxan. With Medicare part D it was "only" $1,100! Guess that's better than the $3,300 price tag I was originally told. That's more than a months groceries by a lot.

Any tips on making it work with the one course? I don't see affording it a second time, and I see some people go through 2 or 3 courses.

H2S SIBO die off with HCL Betaine

I struggled with H2S SIBO for about 3 years, experiencing sulfur smelling breath and stool, bloating, and chronic heartburn.

Things that didn’t work:

  • Rifaximin + neomycin
  • Metronidazole
  • Oregano oil + allicin + NAC

What helped: Betaine HCl + digestive enzymes completely stopped my chronic heartburn.

2 months ago, my SIBO symptoms flared up, so I increased my betaine HCl dosage by two extra pills per meal. This caused burning in my intestines and stool but also fever, chills, and overall dieoff symptoms. After two days, my SIBO symptoms disappeared.

My personal theory is that the increased acid wasn't fully neutralized with my gut's bile --> killed off alot of the overgrown bacterias in my small bowel.

Just sharing my experience! I'm not a doctor, and this isn't medical advice. I do think increasing stomach acid is VERY risky. I am now on back to my normal Betaine HCL dosage
